Health History

Before you meet your chiropractor, one of the team will invite you to fill in a health history form. One of our highly skilled and caring chiropractors will then sit down with you and take the time to really understand your current situation, your state of health and any problems or concerns you might have. Your first visit is designed for us to learn as much as we can about your current state of health, what your goals and expectations are and for us to determine if we can help you.
